merge(#152): suppression grossesse multiple / est_multiple.

Retire le champ partout : BDD (migration + seed), entity/DTO/services Nest,
modèles et payloads Flutter, scripts de test et docs. Back et front alignés
(forbidNonWhitelisted). Aucun fantôme de compat.
